SeniorBridge Helps After the Hospital

Did you know that seniors who are discharged from a hospital have a one-in-five chance of being rehospitalized within a month? For those with chronic conditions such as congestive heart failure or chronic obstructive pulmonary disease the risk is even higher.

Now a new program brought to members by CDPHP and SeniorBridge, a national geriatric care management company, will provide qualifying members the care and attention they need from a specially trained team of SeniorBridge nurses and social workers.

Together, CDPHP and SeniorBridge will support members in making their home a safe place to live, by organizing and overseeing medications, evaluating their medical, emotional, and functional health on a regular basis, and helping them to overcome just about any barriers to adhering to doctors’ recommendations for being well.

"With this program, CDPHP is taking leadership to improve the health and function of its members who are most at risk of being admitted to a hospital as a result of avoidable factors in their homes," said Eric C. Rackow, MD, president and CEO of SeniorBridge.

"We are pleased to partner with CDPHP to expand SeniorBridge's integrated and highly personalized approach to face-to-face care management that is proven to keep vulnerable older adults out of the hospital and safe in the comfort of their homes."

SeniorBridge's professional team will regularly visit enrolled members in the comfort of their homes to identify such risk factors as expired medications and frayed rugs that invite falls, and to identify and address functional and cognitive decline.

Other services available in members’ homes will include health education and counseling, ongoing coordination of care with health care providers and family members, and on-call care management support 24 hours per day, seven days per week.
