Who is SeniorBridge?
SeniorBridge is a care management agency. We provide home-based care to older adults and people with complex or chronic health conditions. Our solutions draw upon the expertise of an interdisciplinary team of healthcare providers, managed by a Geriatric Care Manager. Our model enables our clients to receive care in the comfort of his or her home or whatever setting is preferred. We offer a comprehensive program of care advice, coordination and advocacy, as well a broad spectrum of in-home services, delivered by an integrated, multidisciplinary team of professionals, specializing in the field of aging and chronic care.
Our goal is to help families acheive their goal of providing dignified and safe care in a manner that is consistent with the lifestyle and preferences of the care recipient.
SeniorBridge collaborates extensively with the broader professional advisory community, which includes physicians, accountants, trust managers, bankers, and attorneys. SeniorBridge currently has offices nationwide.
What is Eldercare?
Eldercare helps clients remain and thrive at home – wherever that home may be. SeniorBridge advises clients about alternatives, facilitates change and advocates on behalf of the care recipient. Unlike traditional homecare, which treats only illness, our eldercare model addresses the wellbeing, comfort and functionality of our clients and their families. Our wellness services include enhanced socialization, exercise, cognitive fitness and nutrition management. Our medical services include nursing, medication monitoring and home health care.
