Jill Lebar, MA, LMHC, CMC Executive Director
For 20 years, Jill has been deeply involved in care management and crisis
intervention for frail elderly clients living in the community. She was previously
Director of Creative Alliances, Inc., a large, nonprofit care management agency in
Boston and the Berkshires. She also served as Regional Coordinator of the Caregiver
Alliance, where she collaborated with 4 state elder service agencies and the Boston
Commission on Elderly Affairs and worked on a federally funded caregiver support
program implemented statewide. Jill's other roles included Director of Elder Care &
Work/Life Resource and Referral for Workplace Connections in Waltham and Director of
the Special Care Center at CareMatrix Corp. for Alzheimer's patients in Dedham. She
has a passion for working with individuals and families caring for elders with
Alzheimer's disease and other types of progressive memory loss. Jill has also been a
Full Life Counselor and Care Manager at Genesis ElderCare, Inc. in Boston. She earned
a double BA in Psychology and Studio Art at the University of New Hampshire and a
Master's degree in Art Therapy at Lesley University in Cambridge, MA. She is a
licensed mental health counselor and a certified professional geriatric care manager.
Jill is currently President of the Board of Directors and Chair of the Technology
Committee of the New England Chapter of the National Association of Professional
Geriatric Care Managers.
Charlotte Ann Byrne, RN, BSN, MS Director of Clinical Services
Charlotte has been involved in many facets of home health and senior care since 1989.
Most recently, she was Administrator/Director of Clinical Services at AccentCare Home
Health in California and also Home Health Care Manager and Branch Manager of several
California branches of Professional Health Care at Home. Among many diverse
positions, Charlotte served as Home Care Manager at Centers for Elders Independence
in Oakland and as an independent consultant at Dartmouth College and various assisted
living facilities in New England. She was also Director of Clinical Services at
Caretenders Health Corp. in Newton, Mass. Charlotte held the post of Project Manager
for the Harvard Institute for International Development in Cambridge, managing and
writing grants for numerous health-related international training projects.
Previously, she has also been Special Projects Manager at Boston Senior Home Care and
Director of Home Critical Care at Staff Builders Home Care. Charlotte obtained her BS
in Biology (Pre-Med) from Boston University, a BS in Nursing from Columbia
University, completed credits toward an MBA at Universidad del Pacifico in Lima,
Peru, and earned an MS in Health Policy & Management from the Harvard School of
Public Health.
Gail Schwartz Director of Professional Relations
Gail is a native Californian who relocated to Massachusetts to take on a management
position that included coaching and supporting the sales and marketing teams for 18
nursing homes and solely managing 9. In her previous role with Sava Seniorcare, she
was responsible for educating and maintaining high-level key accounts, working
alongside key executives of managed care plans, hospitals, and large physician
practices in the North East area. Gail's passion for seniors began while working at a
not-for-profit hospital during college and continued in her professional career at
assisted living and nursing home care facilities. Gail earned a BA from the
University of Massachusetts, Amherst studying communications and psychology.
Julie Carmichael, LCSW Care Manager
Julie has been involved in various aspects of eldercare for over years. While she
was Director of Social Services at Orchard Cove, she provided coordinated community
services for consumers and psychosocial support for clients in a sub-acute setting.
She completed her Master's degree , while transitioning from her own geriatric
companion and transportation business, Boston Home Services. She has also worked as a
Licensed Social Worker at Hancock Manor, a family rehabilitation center serving
clients with dual diagnosis, and as a Social Worker/Physical Therapist Assistant at
Park Place Nursing and Rehab and as a Licensed Physical Therapist Assistant at
Wingate at Needham, a rehab and skilled nursing residence. Julie has a Master's
degree in Social Work from Salem State College, Bachelor of Arts degree is in Human
Services from the University of Massachusetts, Boston, and an Associates degree in
Physical Therapy from Massasoit College.
Gail L. Madjzoub, RN Care Manager
For more than 35 years, Gail has accumulated a wealth of experience in diverse
healthcare and educational environments. Most recently, she was Charge Nurse at the
Orchard Cove Skilled Nursing facility and on call for independent residents of the
Orchard Cove retirement community in Canton. She previously served as Education
Coordinator at Kindred Hospital Northeast, organizing and delivering professional
orientation and in-service programs. Gail's experience includes clinical Med-Surg
nursing in ICU, Neurosurgical ICU, and Long-Term Care/Skilled Nursing/Rehab. She is
also a Certified Legal Nurse Consultant and Certified Professional Life Coach.
Earlier in her career, Gail worked in Switzerland as Office Administrator for the
Baha'i International Community and educated high school and university students in
Zimbabwe. She received her RN from New England Deaconess Hospital School of Nursing
and a BA Hons in Modern Languages, Post-Grad Diploma in Translation, from the
University of Zimbabwe.
Dena L. Salzberg, RN Consultant
Dena has been working in the geriatric, home care, and care management field for
more than 25 years. She is the President and founder of Care Management Consultants
of Newton, MA, a company that specializes in the coordination of care for individuals
who have complex and chronic health conditions and is in partnership with
SeniorBridge. Previously, Dena was Vice President of Community Elder Services for
International Health Specialists. She has also served as a Public Health Nurse for
the Town of Braintree and as a Resident Care Coordinator at Hebrew Rehabilitation
Center for the Aged. She has held many leadership positions in the community, which
include Chairperson and Commissioner for the Newton Council on Aging, Past President
and Board member of Springwell (an elder service not-for-profit provider), and Board
member and Trustee of the Maimonides School, Brookline, MA. Dena has written numerous
articles on care management issues, and holds a Bachelor's degree in Nursing from
Boston College.
Ruth Farrago, CMC Vice President of Regional Operations
Ruth Farrago has over 20 years of experience in health care administration. She was the Administrator of an inpatient and outpatient comprehensive Rehabilitation and Orthopedic Division at Downstate Medical Centers in Brooklyn. She has an extensive background in home care marketing and management having worked for St. Vincent’s Catholic Medical Centers as the Director of Intake and was then the Branch Manager/Administrator at New York Health Care. Ruth was also the Executive Director of Sunrise Assisted Living of East Meadow where she was responsible for the daily operations of the facility, client and family satisfaction, management of the sales and marketing team and coordinated community outreach and events. Ruth has a Bachelors of Science from SUNY College of Oneonta where she majored in Business Economics and she is currently completing a Masters Degree at Long Island University specializing in Gerontology. Ruth has received her certification in Geriatric Care Management (CMC).

Internationally Known Harvard University Physician to Collaborate With Firm’s Boston Office to Develop Protocols to Improve Health &Quality of Life
BOSTON, June 3, 2008- SeniorBridge, a specialized organization that serves seniors and their families by managing complex, chronic care in the home and other settings, announced that Juergen Bludau, M.D. will serve as Physician Advisor to its Boston area clinical team of geriatric care managers, nurses, and social workers.
An internationally known expert on geriatric medicine and chronic care, Dr. Bludau is Director of the Geriatric Ambulatory Care Center at Brigham and Women's Hospital, Boston, and also serves as Co-Director of the Annual Harvard Geriatric Review Course, Harvard Medical School.
